For our planning needs regarding space and snacks, it would really help us to know whether you think you might attend the event. On a scale of one to ten, where are you in terms of attending this event? One = I think this is a great idea, but there is pretty much no way that I am going to be able to make it, even though I really want to. Ten = Absolutely, yes. I will be there.

This section is for people who are not community members.

Part of the way we are managing the space is by having a space-holding choir. The space-holding choir will sing members into and out of each QSA, and will take part in keeping the safe space from people who do not understand and/or are protesting its need. If you are not a member of the LGB2TQ+ community, would you be willing to support the event by being involved in our space-holding choir?

I have heard about systemic oppression and agree that it exists.

I agree that members of our community who are more vulnerable than others, generally are so because of the way systemic oppression functions in society.

Do you sit or have you sat on the board or hold personal membership with any other community-serving organizations in Calgary.

In your experience as a board or personal member with a local community-serving organization; have you been involved in community conflict related to power.

Please let us know if there are any supports that can be put into place to support your body while in the space. Examples might include: mobility supports, the absence of allergens, others' awareness of medications you might need and how to administer them.

Please let us know if there are any supports that can be put into place to support your mind while in the space. Examples might include: the presence or absence of a particular stimulus, the availability of a quiet room, the presence of a therapist or member of your support network, peer support, an emergency number or plan in place.

Please let us know if there are any supports that can be put into place to support your communication needs while in the space. Examples might include: visual or audio interpretation.

Please let us know if there are any supports that can be put into place to support you socially. Examples might include: access to onsite childcare for younger children, remote access via internet link.

Is there someone in community with whom you are in conflict? Will that conflict need some support so that you may access the space? Are there any legal mechanisms in place, such as Emergency Protection Orders that need to be considered? Do you need support related to friends and/or family members who are either unsupportive or antagonistic of your identity, or related to your attendance at the QSA? Are there groups of people who overtly or passively contribute to threat in your life that you would like support around in order to attend this event?

If you select yes, we will retain this information for a total of one year, each year you will need to fill in a new long-form registration. If your information changes, you can select the individual long-form registration to any QSA event.
